Cognitive Function: How well you can concentrate, comprehend, and remember as well as your ability to reason, problem solve, and make decisions.
Physical Function: The degree to which you engage in aerobic fitness, strength, endurance, flexibility, balance and perform activities of daily living, for example: walking, and climbing stairs.
Social Connections: The extent to which you derive a general feeling of being connected to others and being able to fulfill roles in the community in addition to the ability to carry out social roles, for example, friend, employee, parent, or spouse.
